Metabolic Health & Weight Loss

 

Metabolic health is about more than the number on the scale. Weight, body composition, glucose regulation, insulin sensitivity, inflammation, nutrition, physical activity, sleep, hormones, and lifestyle can all play an important role in overall metabolic health.

When clinically appropriate, GLP-1 and GLP-1/GIP therapies such as semaglutide and tirzepatide may be incorporated into an individualized approach to support weight management and metabolic health. These peptide-based medications influence pathways involved in appetite, satiety, glucose regulation, energy intake, food cravings or “food noise,” and immune and inflammatory signaling. Improvements in weight and metabolic health may also be associated with improvements in inflammatory and cardiometabolic risk markers for appropriate patients.

At Precision Health, medication is only one part of the strategy. We emphasize resistance training, adequate protein intake, hydration, nutrition, micronutrient optimization, sleep, and preservation of lean muscle mass to support sustainable results and overall health. 

The Role Of GLP-1s and GIP Peptides

 GLP-1 and GLP-1/GIP medications act on incretin hormone pathways involved in appetite, satiety, glucose regulation, energy intake, and metabolic signaling. These pathways also interact with inflammatory and immune signaling throughout the body. They work by: 

  • Appetite & Satiety Support: GLP-1/GIP medications influence appetite and satiety pathways, helping many patients feel fuller with less food and experience fewer cravings or “food noise.” This can support sustainable reductions in energy intake and weight when combined with appropriate nutrition and lifestyle strategies.  (Friedrichsen et al., 2021).


  • Glucose & Metabolic Health: These medications support glucose regulation through multiple metabolic pathways and can improve glycemic control and other markers of metabolic health. Treatment is individualized based on the patient's health history, laboratory findings, metabolic risk, and treatment goals.  (Frías et al., 2021; SURPASS-2) .


  • Body Composition & Visceral Fat: GLP-1/GIP therapy can produce meaningful reductions in body weight, total fat mass, and visceral fat. Because some lean mass may also be lost during significant weight reduction, we emphasize adequate protein intake, resistance training, appropriate nutrition, and preservation of lean muscle mass. (Jastreboff et al., 2022; SURMOUNT-1; Look et al., 2025).


  • Reducing Inflammation: GLP-1/GIP therapies influence immune and inflammatory signaling and have been associated with reductions in inflammatory markers such as CRP/hs-CRP, IL-6, and TNF-α in clinical research. These inflammatory pathways are often associated with obesity, insulin resistance, and metabolic dysfunction. Improvements in metabolic health and inflammatory burden may contribute to overall health, physical wellbeing, and cardiometabolic risk reduction, although individual responses vary  (Masson et al., 2024; Masson et al., 2025; Kanbay et al., 2026).


  • Cardiometabolic Health: GLP-1/GIP therapies may improve several cardiometabolic risk factors, including body weight, glucose regulation, blood pressure, and lipid markers. Certain GLP-1 medications have also demonstrated cardiovascular risk reduction in specific higher-risk patient populations (Lincoff et al., 2023; SELECT Trial) .


  • Sleep & Obstructive Sleep Apnea: Metabolic health, body weight, and sleep are closely connected. Improving metabolic health and reducing excess weight may improve sleep-related health, and tirzepatide has demonstrated benefit for moderate-to-severe obstructive sleep apnea in appropriate adults with obesity  (Malhotra et al., 2024; SURMOUNT-OSA)


  • A Comprehensive Approach: Medication is only one part of our metabolic health program. We combine treatment, when clinically appropriate, with nutrition, protein optimization, resistance training, hydration, sleep, micronutrient support, and ongoing laboratory and clinical monitoring to support sustainable long-term health.

The Importance of Resistance Training

 Resistance training is key for maintaining lean muscle mass and enhancing fat loss when using GLP1/GIP peptides.


Scientific Evidence on Resistance Training:


  • A study in the American Journal of Clinical Nutrition found that combining resistance exercise with weight loss interventions helped preserve muscle mass and improve body composition (Slentz et al., 2011).


  • Resistance training stimulates muscle protein synthesis, which helps prevent muscle loss during calorie restriction (Tesch, 2010).


  • Regular strength training boosts metabolic rate, improving fat-burning even during weight loss (Ransdell et al., 2013).


Incorporating resistance training into a weight loss plan ensures that lean muscle is preserved, which helps maintain metabolism and supports long-term weight loss.

Adequate Protein Intake: Key to Muscle Preservation and Metabolic Health

  To optimize the effects of GLP-1s and GIP peptides, adequate protein intake is essential for building and maintaining muscle mass during weight loss.


Scientific Evidence on Protein & Weight Loss:

  

  • Protein-rich diets boost thermogenesis and promote       satiety, both of which help with weight management (Weigle et al., 2005).


  • A study in The American Journal of Clinical Nutrition showed that increasing protein during calorie restriction improved muscle mass retention and reduced lean tissue loss (Paddon-Jones et al., 2008).


General recommendations suggest aiming for 0.8-1.0 grams of protein per pound of ideal body weight to support muscle repair and prevent muscle breakdown.

Micronutrient Optimization: Minimizing Side Effects and Supporting Gut Health

  While GLP-1/GIP peptides are effective for weight loss, they can cause gastrointestinal side effects like nausea or constipation. Micronutrient optimization helps minimize these effects and supports gut motility—the movement of food through the digestive system..

 

  • Micronutrients and Gut Motility:
    • B Vitamins: B1 (Thiamine), B6 (Pyridoxine), B9 (Folate), and B12 (Cobalamin) support the production of acetylcholine, a neurotransmitter that regulates smooth muscle contractions in the intestines. This helps with peristalsis, the process that moves food through the digestive tract (gut motility). Optimizing B vitamins may reduce constipation, a common side effect of GLP-1/GIP therapy (Lynch et al., 2017).


  • Magnesium: Magnesium helps relax the muscles in the digestive tract, supporting gut motility and preventing constipation (Zhao et al., 2017).


  • Calcium and Vitamin D: These nutrients regulate muscle contraction and relaxation in the intestines, aiding in digestion and reducing discomforts like constipation (Kuan et al., 2020).


  • Fiber: Fiber-rich foods (like fruits and vegetables) help maintain gut health and ensure regular bowel movements, preventing constipation—a common GLP-1 side effect
